MEA Benefits Trust provides support through a nurse coaching program for those who suffer from asthma, diabetes, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF) or coronary artery disease (CAD).
MEA Benefits Trust members who call 866.596.9812 to engage in ConditionCare earn a $100 gift card from one of more than 150 major retailers. Then they work with their nurse coach to manage their condition and once they graduate from the program, they will receive another $100 gift card.
Members who engage in ConditionCare are also eligible for savings on covered medical services and prescription drugs that help manage the condition.
How to redeem your rewards points for gift cards. Once you’ve earned reward dollars, you’ll get an email from our Health Rewards program partner, Hallmark Business Connections. They’ll tell you how to use your dollars. It’s that easy.
